Instructions to fill form online/offline

Form-6 – Offline Mode

You can download a PDF version of the Voter ID Form 6 by visiting the official Voters’ Services portal. It is Available on the Homepage of the portal. You can fill out the downloaded Form 6 application and submit it to the Booth Level Officer’s (BLO) office to apply for a Voter ID card.

Here are the details that you need to fill in the Form 6 application:

Steps:-

1. State, District and Assembly Constituency Name.
2. Your name and surname.
3. Upload/attach a passport-size unsigned photograph that is 4.5 cm x 3.5 cm, showing a front view of the full face with a white background.
4. Name and surname of any one of your relatives and specify your relationship with the individual.
5. Your mobile number and email ID.
6. Your Aadhaar number.  (Optional)
7. Your age in years and months (as of 1^st January in the current calendar year)
8. Your date of birth and gender.
9. Upload/attach proof of date of birth and proof of residence (address)
10. Select or mention if you have any disabilities and upload/attach the disability certificate.
11. Declaration, date, place and signature.

Form-6 – Online  Mode

1. Go to the official Voters’ Service Portal website “voters.eci.gov.in”or Download the 'Voter Helpline App' from the App Store or Google Play Store.
2. Click on ‘Sign-up’ and enter the required details to register on the portal.  
3. Log in to the Voters’ Service portal by entering your mobile number and password.
4. Under ‘Forms’, click the ‘Fill Form 6’ button.
5. Enter all the information on Form 6, i.e., your state, district, parliamentary constituency, personal details, relative’s details, contact details, Aadhaar details, gender, date of birth, address and declaration.
6.  Upload all the necessary documents on the Form.
7. Click the ‘Preview and Submit’ button. When all the details on the form are correct, click on ‘Submit’. 
8. After this, the system will generate an acknowledgement number, which you can use to track your application.

Form 6 Voter ID Documents Required

Here are the documents you need to submit to file a Form 6 application:

Proof of date of birth, any one of the following:

1. Birth Certificate
2. Aadhaar Card (Optional)
3. PAN Card
4. 10^th Standard Certificate
5. Indian Passport
6. Driving License

 Proof of Address, Any One of the Following:

1. Aadhaar Card   (Optional)
2. Bank Passbook
3. Indian Passport
4. Ration Card
5. Revenue Department's Land Owning records including Kisan Bahi
6. Water / Electricity / Gas connection Bill for that address (atleast 1 year)

Form 6 Voter ID Application Status

To check your Form 6 application status, follow the below-mentioned steps:

1. Go to the official Voters’ Service Portal website “voters.eci.gov.in”or Download the 'Voter Helpline App' from the App Store or Google Play Store.
2. Click on ‘Track Application Status’. 
3.  Enter your application reference number, select state and click ‘Submit’.
4. The Form 6 application status will be displayed on the screen.

Form-8

1. Shifting of Residence
2. Correction of Entries in Existing Electoral Roll
   (Name, Gender, DoB/Age, Relation type, Relative's Name, Address,  Mobile Number and Photo)
3. Issue of Replacement EPIC without correction
4. Request for marking as Person with Disability

1. Go to the official Voters’ Service Portal website “voters.eci.gov.in”or Download the 'Voter Helpline App' from the App Store or Google Play Store.
2. Find the ‘Correction of Entries in Electoral Roll’ section on the main page.
3. Tap on the ‘Form 8’ button. You will be redirected to a new page where you should enter the required details. 
4. Select the option ‘Self’ and click ‘Submit’. 
5. Next, select the option ‘Correction of Entries in Existing Electoral Roll’ and click ‘Ok’.
6. The ‘State, District and Assembly/Parliamentary Constituency’ to which you belong will be auto-filled. Check the details and click ‘Next’. 
7. Provide your Aadhaar number, email and mobile number and click ‘Next’. 
8.  Select the detail that requires corrections or changes. In this case, you need to click on the ‘Name’ tab.
9. Enter the name and the supporting documents required for the name change.
10. Upload the supporting document and click ‘Next’.
11.  Fill in the declaration, enter the captcha code and click ‘Submit’.

Document Required for filling of form 8

(Address Proof )

1. Water /Electricity / Gas connection bill for that address (atleast 1Year)
2. Aadhaar card (Optional)
3. Current passbook of  Nationalized /Schrduled Bank/ Post Office
4. Indian Passport
5. Revenue Departments Land owning records including kisan Bahi
6. Registered Rent lease deed (in case of tenant)
7. Registered sale deed (in case of own house)
8. Pan Card
9. Driving License, Certificates of  ClassX or Class XII issued by CBSE/ICSE/ State Education Boards, if it Date of Birth

Fill Form 6B to get Aadhaar and EPIC.

Link Voter ID and Aadhaar via the Voter Helpline App

Steps:-

1. Go to the official Voters’ Service Portal website “voters.eci.gov.in”or Download the 'Voter Helpline App' from the App Store or Google Play Store.
2. Open the app, click on 'Explore,' and 'Log in.'
3. Tap on 'Voter Registration.' Now, select the 'Aadhaar Number Submission (Form 6B)' option and click on 'Let's Start.'
4. Select 'Yes I Have Voter ID number' and press on 'Next.' Provide your Voter ID number (EPIC) and all the required details.
5. Click on 'Proceed,' type in your Aadhaar card number, authentication location, and registered mobile number. Then, select 'Done.'
6. Review all the details you've entered. Once you're sure everything is correct, tap 'Confirm' to finalize and submit 'Form 6B' for the voter card Aadhaar link process.

Steps to download the digital Voter ID or e-EPIC card are as follows:

1. Go to the official Voters’ Service Portal website “voters.eci.gov.in”or Download the 'Voter Helpline App' from the App Store or Google Play Store.
2. Click ‘Login’. Enter the mobile number, password and captcha code and click the‘Request OTP’ button.  
3. Enter the OTP received on your mobile phone and click the ‘Verify & Login’ button.
4. Click on the ‘E-EPIC Download’ tab. 
5. Select the ‘EPIC No’ or ‘Form Reference no.’ option.
6. Enter EPIC number or Form Reference Number, select state and click ‘Search’. 
7. The Voter ID details will be displayed on the screen. Click the ‘Send OTP’ button.
8. Enter OTP and click on ‘Verify’.
9. Click the ‘Download e-EPIC’ button to download the Voter ID card.
